Passengers are required to fill up an online health declaration form three days before entering Bangladesh. The Civil Aviation Authority (CAA) has issued a circular directing the decision to take effect from April 25.

In the first step after login, the passenger has to give his personal information in English, his name, passport number, gender, date of birth, nationality, e-mail, phone number.
The second step is to provide travel information. At this time the passenger will have to give the full address of the country from which he will come to Bangladesh, his flight number, seat number (not mandatory), where he will stay in Bangladesh.
In the third step, the passenger has to give information about Corona. At this time the passenger has to give accurate information about fever, cough, sore throat, shortness of breath, taste or odor etc. Besides, the passenger has to give detailed information about the dates on which the two doses of vaccine were taken by the organization.
The fourth step is to tick the box of a declaration saying ‘All the above information is true’. In the last step, the passenger has to press the ‘submit’ button.
After filling the form, if you press submit, the health card with QR code will come. You have to print it or keep a soft copy on your mobile.
After arriving in the country, the passenger will have to show the health card with QR code or soft copy on the mobile at the health desk of the health department of the airport.
Until then, passengers had to enter the country by filling out a paper health declaration form prepared by the health department. After entering the country, passengers had to stand in long lines at the counter of the Department of Health before the immigration counter.
